Welcome to the Home of McQuaid Jesuit Boys Lacrosse

The nurse’s office will begin accepting Spring sports triplicate forms starting Tuesday February 6th. Your son can pick up a form in our office or these forms can be found on the McQuaid website. Please remember your son must have an up to date physical exam on file in our office in order to clear him to try out. The physical exam needs to be done with in the last calendar year from the start of Spring sports. If your son has not had a recent physical exam, please schedule one ASAP.
We are asking that these forms be turned in by February 26th.
